Name Change Clinic at Broadway Youth Center. We will be supporting folks with getting the process started for a legal name change as well as assisting with gender marker updates. If you have already gotten a legal name change and need support with updating identity documents, come through!

To be the most prepared: fill out our intake form in our bio, come with clear pictures or scans of your birth certificate and Illinois Driver's License ID, If applying for a fee waiver, be prepared to discuss your income in the past 30 days and in the past year (March 2021).
